The Tracker nurse service is for those patients over the age of 65 who may need assistance with the following:
Coping with long term medical conditions
Regular health monitoring
Support if you live alone
Helping to cope with daily life
Assistance with daily living
Are you regularly in and out of hospital?
General support for yourself as well as family members
Do you feel isolated?
Are you a carer for someone?
Do you need financial advice?
Anyone concerned about someone who is registered with Verwood Surgery can refer them to the Tracker nurse service.

The practice manager is involved in managing all of the business aspects of the practice such as making sure that the right systems are in place to provide a high quality of patient care, human resources, finance, patient safety, premises and equipment and information technology. The practice manager supports the GPs and other medical professionals with delivering patient services and also helps to develop extended services to enhance patient care.
Receptionists provide an important link for patients with the practice and are your initial contact point for general enquiries. They can provide basic information on services and results and direct you to the right person depending on your health issue or query. Receptionists make most of the patient appointments with the GPs and nurses. They also perform other important tasks such as issuing repeat prescriptions and dealing with prescription enquiries, dealing with financial claims, dealing with patient records and carrying out searches and practice audits. Their job is very demanding so please be patient.

Heather Donnan leads the community nurse team who offer a high quality professional and confidential home nursing service to the sick frail or elderly and the terminally ill and to patients who are discharged from hospital. They give help and advice to patients and carers through periods of illness on how to obtain assistance from other agencies e.g. Social Services and organisations offering support in the home. They visit those needing nursing care that are confined to their homes or discharged early from hospital. The team can be contacted on 01202 825353.

The Health Visitor and her team specialise in child & maternity care and are also involved in health promotion at other ages. She can be contacted on 01202 824251 (answerphone if out).
Dorset Healthcare University Foundation Trust (DHUFT) employs Community Nurses, Health Visitors, Child and Family Counsellors who work from within Verwood Surgery.
