The Tracker nurse service is for those patients over the age of 65 who may need assistance with the following:

Coping with long term medical conditions, Regular health monitoring, Medication support, Support if you live alone, Helping to cope with daily life, Assistance with daily living and general support for yourself as well as family members.

  • Are you regularly in and out of hospital?
  • Do you feel isolated?
  • Are you a carer for someone?
  • Do you need financial advice?

Anyone concerned about someone who is registered with Verwood Surgery can refer them to the Tracker nurse service.

Hilary Fuggle is also the practice carers lead.

Practice nurses are qualified and registered nurses. They can help with health issues such as family planning, healthy living advice, blood pressure checks, injections and dressings. The practice nurses run clinics for long-term health conditions such as asthma or diabetes and all three are able to carry out cervical smears.

Healthcare Assistants

Healthcare assistants support practice nurses with their daily work and carry out tasks such as phlebotomy (drawing blood), undertake ECG's and assist with minor operations. They may act as a chaperone when a patient or doctor requests one.
